Assist Life Stage Transition

This is a short and long-term supports that focus on strengthening the 

participant’s ability to coordinate their supports, and to assist them to 

live at home and participate in their community. It includes:

We all transition through life and enter a new stage of life as we grow. 

These transitions can be a challenging for many of us. Hiba Healthcare can 

help with life stage transition that may include starting or changing 

school, moving homes or into supported accommodation, adopting pets, 

changing, or deciding to gain employment and even getting out and about in 

the community more.

We understand these changes can be stressful and we support participants 

to plan for these changes and develop strategies to cope with the stress 

through psychosocial coaching and other community-based assistance. We 

stand with the participants side by side and help them achieve their goals  

both in the short term and long term. We focus is on strengthening the 

participant’s ability to coordinate their supports, and to assist them to 

live comfortably and participate in their community.

We help participants navigate major life shifts like leaving school or 

adjusting to a new living situation. Our experience with participants from 

diverse backgrounds means we are always sensitive to cultural expectations 

and traditions as we help them adjust to their newest chapter. We are able 

to provide support before, during, and after the life change to ensure the 

transition is as seamless as possible.

Our well trained and experienced staff are also competent to assist with

Gardening, and Medication Management.
